            **2017 ALABAMA GOURD SHOW ENTRY FORM**

VENDOR BOOTH APPLICATION

Vendors who wish to reserve their last year’s space must do so by March 31st, 2017. New vendors will be able to select from remaining spaces beginning April 1st, 2017. We regret that we cannot accommodate requests for space reassignments at the show. In order to be assured of a space, all applications and checks must be received no later than September 15, 2017. Booth space will be assigned and confirmation sent as soon as possible. Vendors will be supplied with a total of 3 passes (1 for you, 2 for helpers). Electricity is available on all spaces. Vendor must furnish UL approved cord of appropriate size and length. Any additional lamps or lights will be furnished by vendor.  No refunds given after September 1st, 2017

Box 1953, Cullman, AL 35056.  As a participant in the Alabama Gourd Festival, Oct. 20,21, and 22, 2017; I assume all risks, take full responsibility, and waive any and all claims of personal injury, loss, cost or damage to personal property relating to this event.
